The overall purpose of the Court Coordination Care Manager role is to help individuals live their lives to the fullest by ensuring coordination of care and information for timely access to pre-petition screening, Court Ordered Evaluation (COE) and Court Ordered treatment (COT) and any associated transitions.
Court-ordered therapy is meant to be a catalyst for change, for an individual or family who, is at any time, involved in the legal system and subsequently ordered by a judge or magistrate to participate in therapy with a mental health provider.
Overall, court ordered therapy is a way for individuals in the legal system to facilitate positive solutions for legal dilemmas under the supervision of a mental health professional. Since court ordered therapy will most likely play an important role in an individual’s legal situation.
The Court Coordination Care Manager will work with members involved in the court process.  This position will track, monitor and report on the COT/COE requests. 
***This is a field-based position (up to 70%), with some work at home opportunity.  Monday through Friday, 8:00am-5:00pm (with some flexibility)***

If you are located in the Phoenix or Tucson, AZ area, you will have the flexibility to telecommute* as you take on some tough challenges.

Primary Responsibilities:
  • Complete documentation required to address COE/COT requirements and consumer transitions
  • Assist consumers and their families/caregivers with connections to appropriate psychiatric, medical, and psychosocial referrals and services
  • Identify and remove barriers to procurement, delivery, participation in and success of services
  • Provide supportive follow-up, monitoring and education as indicated
  • Meet with consumers in-person wherever they are located - facilitiesprovider offices, and in homes as appropriate for market
  • Establish and foster positive relationships with interdisciplinary team, participate in medical-behavioral integration activities and discussions
  • Identify high-risk, co-morbid needs of consumers and address in care plans
  • Accept referrals via designated processes, collaborate in evaluating available services, and coordinate necessary psychiatric and community referrals as needed
  • Contribute to treatment plan discussions; routinely attend clinical rounds and other meetings with medical partner or external entities when applicable
  • Participate jointly in rounds, discussions, supervision and staffing
  • Collaborate with providers to determine acuity of behavioral health concerns, barriers and progress; coordinate appropriate services
  • Attend state, professional and association and conference call meetings as needed
  • Participate in committees, educational opportunities and trainings
  • Maintain approved workload and support metrics and outcomes
Ensure coordination of care and information sharing for timely access to:
  • Pre-petition screening
  • Court Ordered Evaluation (COE)
  • Court Ordered Treatment (COT)
Ensure treatment of individuals who are unable or unwilling to consent to treatment.
  • Involuntary pre-petition screening, evaluation, and treatment processes
  • Processes for tracking the status of court orders
  • Execution of court orders, and judicial review processes
  • Ensure provision of medically necessary behavioral health services rendered after the completion of a Court-Ordered Evaluation, DUI and Domestic Violence Offender Treatment
  • Ensure that medically necessary behavioral health services are rendered after the completion of a court ordered evaluation, and specific mental health treatment/care when such treatment is ordered as a result of a judicial ruling
  • Ensure compliance with State recognized tribal court orders for members.
  • Work with the Health Plan on reporting requirements

Required Qualifications:
  • Licensed Master's degree in Psychology, Social Work, Counseling or Marriage or Family Counseling, or Licensed Ph.D., or an RN with 2 or more years of experience in behavioral health.
  • Licenses must be independent, active and unrestricted in the state of Arizona
  • 3+ years of experience in a related mental health environment
  • Demonstrated ability to use a PC in a Windows environment, including email and Microsoft Office applications
  • Access to reliable transportation and the ability to accommodate field-based travel (up to 70%)
Preferred Qualifications:
  • Medical/Behavioral setting experience (i.e. community mental health, managed care organization, or joint medical/behavioral outpatient practice)
  • Dual diagnosis experience with mental health and substance abuse
  • Experience working in an environment that required coordination of benefits and utilization of multiple groups and resources for patients
  • Experience working with low-income populations
  • Experience working with the aged, blind or disabled
  • Managed care and utilization management experience is a plus
  • Excellent customer service, interpersonal and problem-solving skills
  • Strong team player and team building skills
  • Experience with COE/COT
  • Able to function independently and responsibly with minimal supervision
  • Maintain direct and open communication with all levels of the organization
  • Able to handle sensitive issues with peer, members and providers in a confidential manner
  • Demonstrate initiative in achieving individual, team, and organizational goals and objectives
  • Ability and flexibility to assume responsibilities and tasks in a constantly changing work environment
  • Strong oral and written communication skills - specifically telephone skills
