The Optum Behavioral Health Home (BHH) program is designed to create a health home system that improves the health and recovery of our members through greater integration of behavioral and physical health services, linkage to needed supports and services, and support for the consumer and their support system. Behavioral Health Homes can improve consumer outcomes, provide greater accountability and flexibility in care delivery, and improve cost. Optum supports the transformation of BHH providers through oversight and support to promote consumer engagement, ensure appropriate assessment and care planning, and deliver improved quality outcomes. Specifically, the BHH program drives improved performance by collecting and sharing data, providing tools and systems that promote integration and coordination, and providing guidance on program design and processes.
The Behavioral Health Home Coordinator will support the Optum BHH program by completing program operations administrative activities. These include processing and managing documents received, completing data entry, tracking necessary data, outreaching the BHH agencies to obtain needed information, and other administrative duties as needed. You will be using the computer and phone to complete your job duties. You will work closely with the BHH Team Practice Specialists, who are Nurses and Behavioral Health Clinicians, to support oversight of the BHH agencies and provide required State reporting.
