As a Blended Case Manager at Devereux’s Children’s Behavioral Health Services you will work with other dedicated professionals who share your passion for helping individuals in need. This position will give you an opportunity to truly make a difference in someone’s life by advocating and providing problem resolution for needed services to education, housing, mental health, employment, healthcare, legal aid and social skills.
Devereux Blended Case Management aims to help children 2-21 years of age and their families to navigate the world of Social Services. We work collaboratively with family members, funders and service providers to advocate on behalf of the child, ensuring care is coordinated and is effective and responsive to the needs of each child we serve.
The home office is located in Phoenixville, PA however this position also works in the community and visiting the homes of assigned clients. Vehicles are provided for work travel.

Education & Experience: Bachelor's Degree in Sociology, Social Work, Psychology, Gerontology, Anthropology, Political Science, History, Criminal Justice, Theology, Nursing, Counseling, or Education or be a Registered Nurse.
The IDEAL candidate will: Be empathetic, a team player, have excellent written and verbal communication skills, energetic, creative, understand the theories of behavior management and positive reinforcement, and represent yourself as a positive role model. Additionally, you must have a strong passion for working with and helping children and adolescents with special needs and possess a valid license.
Physical Requirements: Significant amounts of walking, sitting, and standing. Some lifting and/or managing resistance required related to applying physical intervention techniques upwards of 75-100 lbs. or more. Some repetitive wrist/hand movements, normal hearing and vision. Occasional use of computers and office equipment. Occasional driving of Devereux vehicles for business purposes. Ability to meet 100% mastery of Safe and Positive Approaches
