The Care Management Support Assistant 2 contributes to administration of care management. Provides non-clinical support to the assessment and evaluation of members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Management Support Assistant 2 performs varied activities and moderately complex administrative/operational/customer support assignments. Performs computations. Typically works on semi-routine assignments.

Responsibilities

The Care Management Support Assistant 2 Decisions are typically focus on interpretation of area/department policy and methods for completing assignments. Works within defined parameters to identify work expectations and quality standards, but has some latitude over prioritization/timing, and works under minimal direction. Follows standard policies/practices that allow for some opportunity for interpretation/deviation and/or independent discretion.

ROLE SUMMARY

Perform in a support role providing clerical, fulfillment and telephonic duties for medical management team(s).  Functions include: generating and processing mailings from various departments; reviewing and distributing returned mail for research; data entry of new cases into care management system; providing telephonic support of the department by triaging and distributing calls appropriately.  Adhere to call standards and guidelines; assist Disease Management clinicians in establishing appointments for beneficiary engagement.

Our Department of Defense Contract requires U.S. citizenship for this position

1. Print and process letters, welcome packets and educational mailings for all of HGB Care Management.  Perform all mail functions including receiving, sorting, distributing incoming/outgoing mail, and researching returned mail

2.  Engage in outgoing telephonic communications for the Integrated Care Team in order to establish appointments for beneficiary engagement.  Assists with inbound telephone calls by providing general program information and routing the caller to the appropriate personnel as needed. Enter appropriate information into the Care Management applications and other required applicable systems.

3.  Perform administrative tasks such as schedule meetings, facilitate webinars, scribe/distribute meeting meetings, and other duties as assigned.  Maintain office files and all educational materials for care management programs.

4.  Assist manager and supervisors as needed.

Required Qualifications

  • High School Diploma
  • Ability to handle high volume calls and customer contacts in a polite and professional manner
  • Good organizational and multi-tasking skills
  • Above average written and verbal skills including organizational abilities
  • Computer experience including data entry
  • Must be available to work 9:00 AM – 6:00 PM EST

Preferred Qualifications

  • Post-Secondary training or education
  • 2-4 years of administrative assist experience
  • Health care/managed care experience
  • Direct or Indirect Military experience a plus
