SC MENTOR is seeking a Clinical Coordinator to provides case management and family support services for children, youth, and treatment foster parents supported by the program; Conduct diagnostic assessments as needed for children/youth referred for services; Work collaboratively with the child/youth and treatment team in the development of individualized plan of care; Creates, coordinates and evaluates services plans through partnerships with stakeholders based on assessed client needs; Provides pre-service introduction and in-service continuing education for treatment foster parents as appropriate per regulatory requirements. Provides ongoing monitoring and consultation to treatment foster parents regarding work with children and youth supported and implementation of service plan; conducts home visits; facilitates support groups; Maintains quality documentation in accordance with corporate and regulatory standards; participates in rotational on-call schedule; perform other assigned duties as needed.
- Master’s degree in Human Service field required, Independently Licensed Professional preferred
- At least two year related experience
- Valid driver's license, acceptable driving record and proof of vehicle insurance
- Must be LMSW (Licensed Master Social worker), LPC (Licensed Professional Counselor), or LISW.
