Description

The Community Program Navigator is a frontline associate with a close understanding of outreach strategies and the community based organizations in their assigned region. This position is responsible for establishing and cultivating relationships with community stakeholders and providers, connecting members with critical community support programs, and facilitating community events and education programs.

Responsibilities

The Community Program Navigator is a frontline associate with a close understanding of outreach strategies and the community based organizations in their assigned region. This position is responsible for establishing and cultivating relationships with community stakeholders and providers, connecting members with critical community support programs, and facilitating community events and education programs.

  • Engages community based organizations, cities, churches, health fairs, key providers and other stakeholders to partner on health-related events/initiatives and promote healthy behaviors through increased community visibility
  • Serves as the liaison between Care Managers/Coaches and community organizations by coordinating linkages or referrals for members to community-based services and programs
  • Provides support during transition of care to community setting
  • Regional point of contact for assistance with locating members in the community who are unable to be reached through phone calls and connecting them to Care Managers
  • Identifies when services are needed but not available and seeks new community partners and/or creative gap-fill strategies
  • Facilitates engagement between members and their primary care physician and encourages the completion of health promotion activities,  including, but not limited to HEDIS measures
  • Attends conferences and/or coordinates public and provider educational events in compliance with Medicaid guidelines
  • Develops and maintains relationships with internal partners including Case Managers, Care Coaches and Provider Service Representatives

               

Required Qualifications

  • Skilled in planning, project management, community engagement and development, and resource mobilization
  • Extensive knowledge of the Community Health Worker function
  • Understands the fundamentals of effective community engagement
  • 2+ year’s previous experience in healthcare or health plan marketing, community engagement or member/patient outreach
  • Beginner Excel skills
  • Beginner PowerPoint skills

Preferred Qualifications

  • Bachelor's Degree
  • Familiarity with state Medicaid guidelines
  • Community Health Worker Certification preferred
  • Engagement with Medicaid enrollees, including those with behavioral health needs and varied health literacy
  • Proven knowledge and understanding of how to remove barriers to care in the Medicaid population

Additional Information

  • Ability to travel within territory 80%
  • Territories Needed (Tallahassee, Orlando, Broward County, and Sarasota/Fort Myers)
