Position Summary

The Patient Care Coordinator (PCC) and is responsible for accurate and responsive intake and scheduling of specialty patient referrals and medications via inbound and outbound calls. The PCC provides customer service to retail pharmacies, patients and physician offices and coordinate the delivery of medications for patients.

Job Responsibilities

  • Model excellent patient care by respecting, engaging and enthusiatically meeting or exceeding all patient and physician needs
  • Adhere to all company, state and federal policies, laws and regulations including HIPAA
  • Document all patient/client/provider contact in appropriate computer system
  • Maintain a neat and clean work environment to ensure company and government requirements are met. Remove out of date/expired medications using proper company procedures
  • Utilize all pharmacy systems to review patient profile, set up delivery, enter changes and establish assessment schedule
  • Explains benefits and financial responsibilities and supplies financial assistance contact information
  • Initiates proper follow-up so patients therapy is not disrupted
  • Follows assessments to ensure patient care is optimized (patient on hand quantity of medicine, correct address, insurance changes, side effects)
  • Ability to probe and problem solve on behalf of the patient/customer, including transferring the call to the correct team member for resolution.
  • Act as a liaison with contracted delivery service to track shipments, re-route shipment if needed.
  • Act as a liason to retail pharmacy locations to make sure prompt ordering and filling on prescriptions to meet patient needs
  • Assist in the execution of all company initiatives/programs and participate in staff meeting
  • Report errors, omissions, incidents to pharmacist
  • Assist team members with tasks to ensure department runs efficiently
  • Have knowledge of all appropriate service and business metrics and assist team on action plans to improve outcomes

Education and Training Required

  • High school diploma or equivalent

Experience Required

  • 1 to 3 years
  • Background in a health related customer service environment with at least two years¿ experience

Experience Desired

  • Insurance knowledge preferred
  • Call center experience preferred
  • Computer knowledge ability to navigate in MS Word, Outlook. Typing skills and data entry

Age Requirement
