To support the recommendation, implementation and facilitation of the delivery of sustained changes in prescribing practice to ensure high quality, safe, evidence-based and cost effective medicines use. To provide specialist pharmacy service and pharmaceutical advice across Lincolnshire. Required to work at any establishment at any time throughout the duration of the contract and the post holder will work very closely with member GP practices to support the delivery of specific prescribing initiatives as part of the QIPP agenda.


Primary Responsibilities:

  • To provide a single point of contact for a CCG / group of GP practices for medicines optimisation

  • To be highly visible, accessible and responsive, building strong relationships, especially with GPs, and becoming a familiar and trusted resource

  • To deliver strategic plans and input technical pharmaceutical expertise as well as ensuring implementation of MMO QIPP plans, practice based support and education and training

  • To work in GP practices, working closely with GPs, other practice staff and members of the primary healthcare team in supporting the development and application of high quality, safe, cost effective, rational and evidence based prescribing

  • To conduct appropriate medication review for identified patients, communicating directly with patients as required

  • Provide pharmaceutical advice on a day to day basis in all aspects of prescribing and medicines management to GPs, nurses, other practice staff, and patients as appropriate. This will include answering queries / acting as an information resource

  • Present information in a non-confrontational manner in one to one situations, small groups and large audience, which on occasion be in adversarial conditions. Articulating outcomes

  • Liaise with local community pharmacists, community nurses, community matrons, care home staff, hospital pharmacists and other secondary care providers as necessary

  • To analyse, interpret and monitor practice prescribing and cost data and make appropriate recommendations, including in relation to areas of prescribing where the practice is an outlier within the CCG

  • To carry out medication reviews, discuss options and make recommendations to prescribers as appropriate. This may include patients in nursing and residential homes

  • Assist in managing the introduction of new drugs and drugs that are discontinued

  • To advise on storage of pharmaceuticals within the surgery

Required Qualifications:

  • MPharm in pharmacy (or equivalent prior to 2002)
  • Registered with the General Pharmaceutical Council
  • Evidence of Continuous Professional Development (CPD)
  • Post registration relevant experience
  • Extensive experience in clinical pharmacy and drug utilisation review
  • Experience of inter - disciplinary working
  • Full driving license and access to transport
  • Willingness to travel within Lincolnshire, and neighbouring areas if required
  • Experience of working in GP practices and primary healthcare teams
  • In depth therapeutic and clinical knowledge and understanding of the principles of evidence based healthcare
  • A sound understanding of the nature of primary care prescribing, concepts of rational prescribing and strategies for the improvement of medicines optimisation overall
  • Understand the concepts of evidence based medicine, needs assessment, effectiveness and prioritisation
  • Understanding of commissioning processes
  • Understanding principles of clinical governance and how these apply to the broader agenda
  • Able to demonstrate excellent organisational skills, and an ability to prioritise and work to deadlines
  • Competent in use of standard IT and relevant software
