Field-Based RN Case Manager - Virginia

If you are located in the state of Virginia, you will have the flexibility to telecommute* as you take on some tough challenges.

 This is a field-based role in the state of Virginia with up to 40% travel.
Primary Responsibilities:
  • Visit members in their homes and/or other settings, including community centers, hospitals or providers' offices
  • Provide a complete continuum of quality care through close communication with members via in-person or on-phone interaction
  • Support members with condition education, medication reviews and connections to resources such as Home Health Aides or Meals on Wheels
  • Assist members with the transition from a care facility back to their home
This is an opportunity to travel and work remotely for a Fortune 5 company. This role involves approximately 40% travel and you’ll need to be comfortable dealing with a full range of in-home and facility-based environments. Most of the time you’ll work autonomously, so the ability to stay focused and motivated is essential.

Required Qualifications:
  • Active and unrestricted RN License in Virginia
  • 3+ years clinical experience in a hospital, acute care, home health / hospice, direct care or case management
  • 1+ years of experience working with individuals with special healthcare needs
  • Computer / typing proficiency to enter / retrieve data in electronic clinical records; experience with email, internet research, use of online calendars and other software applications
  • Access to reliable transportation
  • Familiarity with the culture and the values of the community and the resources available
Preferred Qualifications:
  • Case management experience including Certification in Case Management
  • Home care / field based case management
  • Experience working with the needs of vulnerable populations who have chronic or complex bio-psychosocial needs
  • Experience in Home & Community based or Long Term Care services delivery
  • Familiarity with the culture and the values of the community and the resources available
  • Experience in child placement services through both a legal and behavioral health lens and trauma-informed clinical practice.
  • Experience with Vulnerable subpopulations including children with special health care needs, adults with serious mental illness, children with series emotional disturbances, members with substance abuse disorders, children in foster care or adoption assistance and members with other complex or multiple chronic conditions
  • Experience working in Managed Care
  • Bilingual skills
  • Knowledge of Medicaid and / or Medicare population
  • Must reside in Virginia
