Under general supervision of the Director of Clinical Pharmacy, the Clinical Formulary Pharmacist is responsible for the general oversight of medication management activities for all WellMed clinics to meet our Joint Commission Accreditation and Primary Care Medical Home Certification. The Clinical Pharmacist is also responsible to follow processes to improve patient care with regards to medication management and have oversight of the medication purchasing across all WellMed clinics.

 

Primary Responsibilities:

  • Participate in formulary development and maintenance for the clinics; ensure alignment with health plan’s formulary guidance
  • Provide clinical support and drug information to employed providers
  • Coordinate the development of system-wide standards related to medication use policy across all WellMed clinics
  • Perform periodic drug use evaluation (DUE) to ensure appropriate use of medications
  • Research new prescription drugs or new applications of existing drugs and develops drug use guidelines, and communicate findings to providers
  • Assist in development of Clinical Practice Guidelines
  • Conduct comprehensive review of new drugs or new indications for new drugs, and present findings to the Clinical Health and Safety committee
  • Provide quality oversight to various programs to ensure regulatory compliance and continuous process improvement
  • Provide clinical oversight and guidance to clinical staff regarding medications
  • Create policies and procedures regarding medication samples, stock medications, and medication administration within the clinics
  • Develop educational materials and mentoring training programs within area of expertise
  • Identify opportunities for improvement in medication management and clinical quality
  • Utilize prescription and medical claim data to generate clinical recommendations according to Medication Therapy Management and Retrospective Drug Utilization Review program protocols
  • Provide clinical recommendations pertaining to, but not limited to, gaps in care, high risk medications, compliance and adherence, drug interactions, therapeutic substitution, and generic substitution
  • Knowledgeable of current and emerging clinical trends
  • Active participation in various clinical departmental meetings and forums to improve clinical programs, enhance processes, and share clinical information
  • Continuously look for ways to streamline and improve workflow
  • Assist with user acceptance testing for new clinical programs and system enhancements
  • Serves as a clinical resource and consultant in all areas of pharmacy and medication management
  • Performs all other related duties as assigned to meet departmental objectives

Required Qualifications:

  • Doctor of Pharmacy degree
  • Current Pharmacist license to practice in the applicable state or the ability to provide proof of licensure within 90 days from date of employment
  • 2+ years of experience as a clinical pharmacist  or similar experience that includes a residency in Pharmacy
  • Experience in formulary development and maintenance
  • Experience creating and adhering to health plan formulary guidelines
  • Reliable transportation that will enable you to travel to client and/or patient sites within a designated area
  • This position requires Tuberculosis screening as well as proof of immunity to Measles, Mumps, Rubella, Varicella, Tetanus, Diphtheria, and Pertussis through lab confirmation of immunity, documented evidence of vaccination, or a doctor’s diagnosis of disease

Preferred Qualifications:

  • Board certification by the Board of Pharmacy Specialties (BPS)
  • Certified Geriatric Pharmacist
  • Certification in Medication Therapy Management (MTM)
  • Experience with Medicare guidelines
  • Excellent written and verbal communication skills
  • Excellent relationship building skills
  • Planning and organizational skills to demonstrate leadership and initiative
  • Strong interpersonal skills. Ability to work independently, as well as a member of a team
  • Strong analytical and PC operation skills

Physical & Mental Requirements:

  • Ability to lift up to 25 pounds
  • Ability to use fine motor skills to operate office equipment and/or machinery
  • Ability to receive and comprehend instructions verbally and/or in writing
  • Ability to use logical reasoning for simple and complex problem solving
