Manager RN - Hospital Care Transition - Missouri

Be an integral part of a vital team who is working to ensure adequate and appropriate care transitions from a hospital setting to the next level of care. Interact with a team of coordinators and facilities to drive positive experiences for the member and provider.
Ensure smooth internal and external handoffs and partnerships across the enterprise and to support a member-centric approach to care transitions within the local market.
You’ll enjoy the flexibility to telecommute* from anywhere within the U.S. as you take on some tough challenges.
Primary Responsibilities:
  • Takes lead role in setting direction and participating in or developing new programs
  • Sets team direction, resolves problems and provides guidance to members of the team
  • Ensures team meets established performance metrics and performance guarantees
  • Adapts departmental plans and priorities to address business and operational challenges
  • Contact assigned hospitals to obtain credentialing requirements for coordinators
  • Assist with hiring process of HCT coordinators

Required Qualifications:
  • Active Unrestricted RN License for your state of residence
  • 3+ years of experience as Case Manager
  • 2+ years' of clinical management experience overseeing a staff of 10 or more
  • Efficiently navigate Microsoft Office (Word, Excel, Outlook)
  • Ability to be credentialed at assigned hospitals and meet all hospital occupational health requirements (drug screening, licensure and immunizations)
  • Data entry experience into case management systems
Preferred Qualifications: 
  • BSN or MSN
  • A background in managed care
  • Complex Case Management experience
  • Certified Case Manager (CCM)
  • Experience / exposure with discharge planning
  • Experience in utilization review, concurrent review and / or risk management
  • Strong organizational skills and multitasking abilities
  • 3+ years' of clinical management experience overseeing a staff of 10 or more
