Nurse Practitioner - UHC Clinics, Tempe, AZ

We are looking for a Nurse Practitioner (NP) to join our team and help deliver on our mission to improve access to care.  The NP will work in the Papago neighborhood in Tempe.

Primary Responsibilities:

  • Lead patient care as a primary care provider in the UHC neighborhood clinic
  • Conduct physical assessments to diagnose and treat patients for common acute injuries, infections, and illnesses within the confines of licensure
  • Counsels and educates patients and families about illnesses or concerns
  • Formulates, implements and continuously evaluates diagnostic and treatment plans
  • Prescribes and administers medications and therapies as needed
  • Ability to work within a collaborative clinical environment that services patients across the lifespan and with a wide variety of diagnoses and/or chronic conditions
  • Proactively identifies opportunities for efficiency and care delivery improvement and leads creation of the protocols
  • Communicates and supports quality clinical outcomes with the larger clinic team via continuing education and adoption of best practices
  • Passion for improving care delivery through testing and applying innovative new operational and clinical models
  • Demonstrate passion for serving the underserved

Required Qualifications:

  • Graduate of an accredited Master's Degree in Nursing (MSN)
  • Current, unrestricted NP license in Arizona in good standing
  • Board Certified through the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC) in Adult, Family or Pediatrics
  • 2+ years of post-graduate, experience working as an NP in an Adult, Family, or Internal Medicine setting
  • Comfortable treating patients across the lifespan and with a wide variety of diagnoses and/or chronic conditions
  • Advanced knowledge of and experience with symptom management
  • Computer literate and experience with Electronic Medical Records
  • Current DEA license or ability to obtain DEA licensure / Prescriptive Authority in Arizona
  • DEA x-waivered or willingness to obtain that waiver upon hire to support medication assisted treatment (8 hour course needed to do this)

Preferred Qualifications:

  • Experience working in a large primary care group practice
  • Prior experience building a practice
  • Prior experience providing max licensure level of care to low income populations
  • Bilingual in Spanish. Spanish Medical terminology a plus
  • Entrepreneurial spirit and passion for improving the patient experience
  • Integrated care model experience
  • Familiarity with Vaccines for Children program
