The Patient Care Coordinator in Basking Ridge,
NJ in this function is part of an interdisciplinary care team of
palliative care providers to support patients facing serious illness, including
their families and care givers. As part
of the team, patient care coordinators play a critical role in helping to
coordinate care for palliative care patients, including community resources,
hospice, home health and durable medical equipment, along with facilitating the
communication of patient medical records and visit notes with collaborating
healthcare providers and partners.
Additional responsibilities may include support of patient outreach and
engagement, patient scheduling and rescheduling, and other administrative
functions, as required.
