The Patient Care Coordinator in Basking Ridge, NJ in this function is part of an interdisciplinary care team of palliative care providers to support patients facing serious illness, including their families and care givers.  As part of the team, patient care coordinators play a critical role in helping to coordinate care for palliative care patients, including community resources, hospice, home health and durable medical equipment, along with facilitating the communication of patient medical records and visit notes with collaborating healthcare providers and partners.  Additional responsibilities may include support of patient outreach and engagement, patient scheduling and rescheduling, and other administrative functions, as required.

Primary Responsibilities:

  • Participate in weekly interdisciplinary care team meetings, including prepare patient census report, take minutes, and maintain an accurate log of follow - up items

  • Perform outreach calls to patients to enroll in palliative care

  • Coordinate care for palliative care patients, including community resources, hospice, home health and durable medical equipment

  • Facilitate the communication of patient medical records and visit notes with collaborating healthcare providers and partners

  • Answer the telephone during regular business hours to response to patient, family and provider inquiries and triage to appropriate team members as appropriate

  • Schedule initial patient visits, pre - visit reminder calls and support rescheduling, as needed

  • Conduct patient satisfaction surveys for performance measurement

  • Assist with mailing educational resources and marketing materials to patients and families, upon request

  • Office budget management and oversight, including support for clinician travel expenses

  • Document in the clinical management system

  • Sort all incoming mail for distribution, dispatch outgoing mail, and re - address undeliverable mail bearing incomplete or incorrect address.

  • Run reports

  • Other administrative duties as defined

Required Qualifications:

  • High School Diploma / GED or higher

  • 1+ years experience with patient clinical documentation systems (electronic medical record)

  • 1+ years of medical terminology

  • Proficient with Microsoft Word (creating and editing documents), Microsoft Excel (general spreadsheet navigation) and Microsoft Outlook (email and calendar)

  • Will be required to attend 2 - week training

Preferred Qualifications:

  • Experience in palliative care or hospice
  • Bilingual or multilingual fluency, including the ability to read, write & speak
