Position Summary

The purpose of this position is to assist the manager in the development, leading, and coaching of a clinical pharmacy team who will work to improve patient adherence and address any identified medication therapy omissions. This will be accomplished through outreach phone calls, as prompted by the MTM platforms utilized by Giant Eagle Pharmacy.

Job Responsibilities

  • Assist the manager with directing, educating, and executing Medication Therapy Management for the company. This includes but is not limited to Mirixa, Socrxates, and OutcomesMTM platforms. MTM sessions will focus on adherence gaps and therapy omissions but can also include Comprehensive Medication Reviews (CMRs), Targeted Intervention Programs (TIPS), Gap-in-Care Alerts (GAPs), and ESI Star Ratings Programs.
  • Complete required training for disease state management and maintain an up-to-date competency regarding therapeutic interventions.
  • Address adherence and omission gaps while reiterating the clinical significance of being compliant.
  • Schedule and perform follow-up adherence calls to ensure success and continued compliance with medication.
  • Provide adequate notes and documentation within platform to ensure proper reimbursement, patient safety, and to provide a seamless management of patients amongst all staff.
  • Train and educate staff on proper clinical conversations, documentation, and scheduling timeline based on platform being used.
  • Demonstrate adherence to all company, state, and federal policies, laws, and regulations including HIPAA through example and coaching.
  • Develop and maintain good working relationships with clinical pharmacy staff and personnel as well as motivating clinical pharmacy staff to generate an environment that ensures the attainment of department goals and objectives.
  • Develop and maintain good working relationships with physicians, office staff, and associated health care professionals.
  • Assist manager to provide an optimal team and patient experience by proper delegation of work assignments and efficient use of supplies and equipment.
  • Review all appropriate service and business metrics and assist team on action plans to improve outcomes.
  • Maintain a neat and sterile work environment to ensure all company, local, state, and federal requirements are met.
  • Precept/Mentor assigned residents and/or interns. Involve students in assigned activities, working with them to achieve mutually beneficial goals. Accept IPPE and APPE students for experiential rotation.
  • Perform additional duties as assigned.
