TeamMD is a new model of care focused on transforming care for high-risk Medicare and Medicaid patients in their home.  We are currently expanding our team in Tennessee, and we are looking for a practicing Psychiatrist who can bring compassion, behavioral health and addiction expertise, energy, and focus to this work.

 

In this role, you will serve as a consultant to practitioners in the field who are completing in - home assessments of chronically ill and high-risk patients in a variety of environments including private homes, assisted living facilities, nursing homes, skilled nursing facilities, and / or hospital settings.  You will also provide consultation for the post-acute stabilization program as well as provide support for clinicians as they care for patients transitioning from higher levels of care.  You will collaborate with a multidisciplinary team to engage resources and strategies to address medical, behavioral, functional, and social barriers to care.  Additionally, the successful candidate will be involved in building out clinic-based (1) medication assisted treatment and (2) outpatient programs for individuals with serious and persistent mental illness.

 

You’ll enjoy the flexibility to work from home in TN and must be available to attend in-person leadership meetings twice a month at the Brentwood, TN office. This opportunity offers a competitive salary and a $50k sign-on bonus! 

 

Primary Responsibilities:

  • Provide consultative support for TeamMD physicians, nurse practitioners, nurses, and social workers on complex behavioral health and psychotropic medication issues
  • Review, interpret, and integrate data to guide best practices in planning and delivery of behavioral health care
  • Provide direct psychiatric care and telemedicine psychiatric consultation depending on the needs of TeamMD patients  
  • Assess risk potential, provide and supervise crisis interventions, triage phone calls, and prescribe psychotropic medications as needed
  • Complete full and appropriate documentation to support professional billing for services
  • Provide education regarding illness trajectory and prognosis to assist with clarification of patient and family goals of care and support development of a comprehensive care plan.  
  • Engage with community-based primary care and behavioral health providers to advocate for appropriate treatment and services when needed
  • Provide input to organizational quality initiatives and develop ongoing staff education
  • Develop guidelines and strategies to identify and manage complex behavioral health, addiction,, and co-occurring service needs for TeamMD patients
  • Participate in collaborative multidisciplinary team meetings and case reviews to optimize clinical integration, efficiency, and effectiveness of service delivery

Required Qualifications:  

  • Board Certified in Adult Psychiatry
  • Active and unrestricted medical license as a Doctor of Medicine (MD) or Doctor of Osteopathy (DO) within Tennessee
  • Residency in the state of Tennessee
  • 5 or more years of clinical practice
  • Current DEA licensure and prescriptive authority in state where applicable
  • Ability to work with diverse care teams in a variety of settings
  • Effective time management and communication skills
  • Advanced knowledge of and experience managing complex psychiatric conditions
  • Computer literate and experience with Electronic Medical Records  
  • Willingness to commute up to 25% for face to face meetings and conduct in-home visits within a 75 mile radius from home office in Tennessee 

Preferred Qualifications:

  • Excellent administrative, organizational and verbal skills
  • Effective communication skills
  • Experience working with geriatric, Medicaid, and/or Medicare populations
  • Experience supervising or providing supervisory oversight to clinical staff
  • Experience with telemedicine or delivering care from an off-site location
