Our Registered Nurse, Community Liaison works within the scope of Ohio Nurse Practice Act, will provide care in an atmosphere sensative to each person's physical, emotional, social and spiritual needs. The Registered Nurse, collaborates in the spirit of teamwork is accountable for the delivery of patient care utilizing the nursing process within the standards, policies, procedures and guidelines of the Organization.

Responsibilities   

  • Interacts with community partners  along with hospice and palliative care stakeholders in the system to increase awareness and promote program clinical expertise and services.
  • Acts as representative of the program to various community groups including but not limited to, patients/families, community providers, medical staff, volunteer organizations, residential care facilities, aging services, and primary care physicians to educate regarding accessing the programs and help improve transition between providers    
  • Assists in the identification, development, and implementation of new programs/initiatives in order to meet patient needs in their care setting

Requirements/Qualifications

  • Education: Associates degree from an accredited RN program
  • Current Ohio Registered Nurse (RN) license
  • Ohio, Must have a valid Driver's License
  • Minimum of 2 years' experience in Hospice or Homecare
  • Criminal Fingerprinting as required by Ohio law
  • BLS Healthcare provider training required
