Senior Patient Care Coordinator - Rocky Hill, CT

The Senior Patient Care Coordinator will be tasked with transitioning information from Microsoft Excel spreadsheets to the EMR database; This is patient information. You will also receive information from the Nurse Practitioners related to a patient's level of care, and then you will have to apply that to a criteria and see what level of care is needed. You will then enter this information into the system and communicate it to the Nursing facilities via fax. There is a lot of Data Entry involved in this position. Other duties will be assigned, as needed.


Primary Responsibilities:

  • EMR management, including patient assignment and report management
  • Assignment of Clinical Team and Nursing homes for our ISNP program and our TTS program
  • Benefit determination; processing of Excels and utilization of ICUE and EMR to manage benefits of members
  • Communication with skilled nursing facilities
  • Management of Key clinical processes for licensing, equipment, training.
  • Scheduling of appointments with key stakeholders, room reservations

  • Supporting the interview and hiring process with resume management, filing of paperwork

Required Qualifications:

  • High School Diploma / GED or higher

  • 6+ months of professional experience in a medical setting

  • Knowledge of Microsoft Office, including the ability to sort, filter, and document information using Word, Excel, & Outlook

  • Must be able to type 30+ WPM

  • Previous work experience using medical terminology

Preferred Qualifications:

  • Customer Service experience
  • Data Entry experience
