The Hospice Medical Social Worker provides direct casework and counseling services to patient/family unit. Provides diagnostic evaluation of psychosocial problems and defines social service goal for alleviating problems. Provides discharge planning for appropriate patients and families. Serves as part of interdisciplinary team and coordinates patient care with primary nurse. Participates in the provision of bereavement services. Assists with Hospice Home patients when needed.

Required Licenses/Certifications

  • Valid SC Social Work License
  • Valid Driver's License
  • Maintain CPR Certification
Core Job Responsibilities

  • Evaluates and revises plan of care appropriate to current and collaborated assessment.
  • Demonstrates current knowledge of community resources appropriate to setting and population. Refers to community resources as needed.
  • Performs assessment, teaching, advocacy and linkage with community resources.
  • Demonstrates effective assessment skills in identifying abuse and neglect, effective use of team to collect data, and demonstrates referral to appropriate intervening agency (i.e. Adult Protective Services) as necessary.
  • Shows competency in assessing and intervening in patient/family adaptation and functional state (A person's ability to emotionally or spiritually adjust to changing environmental conditions or life circumstances)
  • Performs assessment, teaching advocacy and linkage with long-term care options for patient.
  • Facilitates agency contacts via phone or direct for patients/families requiring other forms of assistance, i.e., program entitlements.
  • Interfaces with area nursing homes, extended care facilities, and hospital to facilitate transfer of patients, when appropriate.
  • Participates in inter-disciplinary conferences and staff meetings.
  • Responsible for informing team regarding patients and families end of life decisions and changes.
