Telephonic After Hours Part Time RN On Call - Telecommute in FL, GA, or NC

This position is Part time working 28 hours a week.  You must be available to work every other weekend.If you are located in Georgia, Florida, or North Carolia, you will have the flexibility to telecommute* as you take on some tough challenges. 


Primary Responsibilities:   
  • Making outbound calls to facilities to discuss members current health status        

  • Care Coordination for members        

  • Assist the provider / team with various care coordination activities to support the role of the On Call Nurse Practitioner        

  • Responsible for collaborating with the nursing facility and community members to identify and respond to changes in condition        

  • Assist the provider / team with inbound and outbound communications for members including telephonic assessments or acuity follow - ups        

  • Conduct telephonic rounding and identify high risk patients or issues for the Nurse Practitioner to follow - up

  • Document all clinical information and activities in the EMR from telephonic assessments    

Work from home a combination of:
Weekdays: 5 pm - 8 am
8 am - 8 pm & 8 pm - 8 am weekend & holidays (Saturdays and Sundays) - OVER NIGHT (must be awake)
PLUS rotating holidays
Must work 4 weekend shifts a per month

Required Qualifications:  

  • Current, unrestricted RN license in either NC, GA, or FL

  • 3+ years direct care clinical experience in a hospital, acute care, home health/hospice  

  • 6+ months on call experience as a nurse

  • Dedicated space for home office set up  

  • Access to high speed internet services  

  • Proficient with Microsoft Word, EMR and ability to navigate a Windows environment

Preferred Qualifications:  

  • Case Management experience including Certification in Case Management  

  • Experience working with the needs of vulnerable populations who have chronic or complex bio-psychosocial needs  

  • Experience triaging calls  

  • Managed Care experience  

If the hired individual resides in Florida (office based or telecommuting) this position requires the AHCA Level II background check (fingerprinting) by the State of Florida for all clinicians that have direct face to face contact with members OR employees who will have access to confidential patient data and will require renewal every five years.
